Supporting GPs to address the social determinants of diabetes

We would like to invite you to participate in a study exploring how primary care staff (including GPs, practice nurses, health workers and others) can help people with diabetes to address the social factors which affect their life and health.

 We want to learn about your experience of screening for and helping to manage social  factors in your daily practice, what things help or make it more difficult and how we can equip primary care staff to support their patients in these challenges. 

 

To do this, we are inviting any staff employed at an Australian general practice to
